The aim of this study was to evaluate the effect of gonadotrophin-releasing hormone (GnRH) antagonists on the hypothalamic-pituitary-gonadal (HPG) axis and to investigate the potential effect of clomiphene citrate on this axis. The present study was a prospective, controlled, randomized, single-center trial conducted in a tertiary center in the Philippines between January and December 2014.
We hypothesized that, in addition to being a selective hypothalamic-pituitary-gonadal (HPG) axis agonist, gonadotrophin-releasing hormone (GnRH) antagonists could also induce HPG axis in the hypothalamus. A total of 431 female patients with a body mass index (BMI) between 30.7 and 40.9 kg/m2 (mean 35.9 ± 7.6 kg/m2), with a mean age of 33.5 ± 8.3 years were recruited to the study. All patients were evaluated before starting treatment and after 3 months and then on the following 3 months. Patients with an BMI of 30.2 or higher were eligible for the study. Patients with a BMI of over 40 kg/m2 were excluded as this group did not have sufficient evidence of HPG axis in patients with hypogonadism.
The patients were followed up every year for a period of 1 to 3 months (median 6 months).
Patients in the study group were treated with clomiphene citrate (50 mg) at a dose of 1 mg/day for 1 week. For the first month of the study, the patients were administered clomiphene citrate (25 mg/day, 50 mg/day and 100 mg/day). The first month of the study was the first month of treatment, and the treatment was repeated every 3 months. For the second month, the patients were administered the following treatments: clomiphene citrate (25 mg/day, 50 mg/day, and 100 mg/day), which resulted in a reduction of the mean of the first three measurements. The last treatment was a second treatment (clomiphene citrate 25 mg/day, 50 mg/day, and 100 mg/day), which resulted in a reduction of the mean of the three measurements.
For the third month of the study, the patients were administered clomiphene citrate (25 mg/day, 50 mg/day and 100 mg/day), which resulted in a reduction of the mean of the first three measurements. The patients were instructed to keep on clomiphene citrate (50 mg/day, 50 mg/day and 100 mg/day) for the remainder of the study period, and then to take the study medication for the remainder of the study period.
All patients were instructed to follow up the follow-up visits every year for 1 year (1 year after their last treatment with clomiphene citrate). The patients were followed for a period of 7 years (4 years after their last treatment with clomiphene citrate).
The patients were instructed to continue the study treatment after the end of the treatment period and continue taking clomiphene citrate for the rest of the study period (6 months after the end of treatment).

The study medication consisted of a placebo, with clomiphene citrate (50 mg) taken as a placebo (the dose was 25 mg/day). The patients were instructed to continue the study treatment after the end of the study period. The study medication was continued after the end of the study period.
